All full-time Park Pro employees are eligible for Medical, Dental and Vision benefits on the first of the month following 60 days of employment.
Dependent children are eligible through the end of the month following their 26th birthday.
HOW AND WHEN CAN YOU MAKE CHANGES TO YOUR ENROLLMENT STATUS?
Open enrollment will occur once a year. Each year you may change your coverage level. Once you have made your selection, you may not change it until the next year's open enrollment unless you have a "life changing event." Life changing events include:
Change in marital status
Change in the number of dependents
Change in employment status
Dependent's satisfying or ceasing to satisfy dependent eligibility requirements
Significant Cost Changes
Significant Curtailment of Coverage
Addition or Improvement of Benefit Package Option
Change in Coverage of Spouse or Dependent under another Employer Plan
Loss of other health coverage
Judgment, decree, or order
Entitlement to Medicare or Medicaid
In the event that you have a "life changing event", you must complete the necessary paperwork and notify the Administrator within 30 days if you wish to change the status of your insurance coverage.
